Part 7045.0230 - CONTENT OF INITIAL LICENSE APPLICATIONSubpart 1.Information required.Except as provided in subpart 1a, an application must be on a form provided by the commissioner and must include the following information:
A. the generator's company name, location address, mailing address, type of business, principal products or service, contact person, telephone number, and identification number or date applied for;B. a list of all hazardous wastes generated, their corresponding hazardous waste numbers from parts 7045.0131 and 7045.0135, and the physical state, and the source or process from which the wastes are generated;C. a list of all used oils generated, the physical state, and the source or process from which the waste was produced;D. a management plan for each hazardous waste and used oil produced that includes the following information: (1) the amounts produced in the previous calendar year;(2) the name and identification number of the most frequently used transporter;(3) the names and identification numbers of the designated facilities involved in the management of the hazardous waste;(4) the methods of management, on and off-site, proposed for each hazardous waste; and(5) the year each hazardous waste was first produced;E. the following certification signed by the generator or authorized representative: "I certify under penalty of law that I have personally examined and am familiar with the information submitted in this and all attached documents, and that based on my inquiry of those individuals immediately responsible for obtaining the information, I believe that the submitted information is true, accurate, and complete. I am aware that there are significant penalties for submitting false information, including the possibility of fine and imprisonment."; and
F. any additional information regarding the generator or the waste produced and managed by the generator which is necessary to a decision on the application and which has been requested by the commissioner.Subp. 1a.Very small quantity generator license application.An application for a very small quantity generator license must be on a form provided by the commissioner and must include the following information:
A. the generator's company name, location address, mailing address, type of business, contact person, telephone number, and identification number;B. the date each hazardous waste was first produced;C. the name or a description of each hazardous waste generated;D. the estimated amount of each hazardous waste to be produced in a one year period;E. the methods of treatment and/or disposal;F. the certification required under subpart 1, item E; andG. additional information required under subpart 1 as requested by the commissioner.Subp. 2.
