Part 4731.4511 - SURVEY RECORDS; SOURCE IMPLANT AND REMOVAL

A licensee must maintain a record of the surveys required under parts 4731.4451 and 4731.4464 for three years. The record must include:

A. the date and results of the survey;

B. the survey instrument used; and

C. the name of the individual who made the survey.
