Section R. 338.539 - Relicensure and renewalRule 39.
(1) An applicant with an expired license may apply for relicensure of a pharmacy license by submitting to the department a completed application on a form provided by the department, satisfying all the requirements for licensure in part 3 of these rules, R 338.531 to R 338.539, and paying the requisite fee.(2) A pharmacy that renews its license during the license renewal period submit to the department a completed application, on a form provided by the department, together with the requisite fee.Mich.
