Rule 121. Ownership, whether by the individual desiring to establish, conduct, or maintain a licensed health facility or agency, or by the authorized representative of an individual, copartnership, corporation, or association desiring to establish, conduct, or maintain a health facility or agency, must be disclosed to the department upon initial licensure application.
