(1) There are no specific requirements relative to the form of documents.(2) A document shall contain: (a) A caption sufficient to identify the parties.(b) The board's docket number, if assigned.(c) A clear, plain statement of the relief sought, together with the grounds therefor.(d) A signature by the party involved or by his representative.Mich.
