Section R. 325.2115 - Operation permit application

Rule 5.

(1) An applicant for an operation permit shall submit an application to the department on the forms prescribed and provided by the department.

(2) An applicant shall submit the fee prescribed by section 12527a of the code together with the application. An applicant shall make payment payable to: "State of Michigan."
