Section R. 325.1701a - Proof of work experienceRule 201a.
(1) Proof of an applicant's work experience in the well drilling or pump installation field shall be documented by submitting all of the following to the department: (a) Well logs or pump records to demonstrate completion of not less than 20 water wells by a well driller applicant and not less than 20 pump installations by a pump installer applicant. The records shall be true and accurate photocopies of those records initially submitted by the supervising contractor to satisfy the requirements of section 12707 of the act. The records shall demonstrate completion of work by the applicant over a period of not less than 2 years and not more than 5 years.(b) A chronological work history, on a form furnished by the department, that documents all work performed in the well drilling or pump installation field. Before submitting the work history form to the department, an applicant shall submit the work history form for review to the local health department that has jurisdiction in the area in which the applicant has worked.(c) Notarized reference letters, on a form provided by the department, from not less than 2 persons who are not related to the applicant and who can attest to the applicant's work experience and ability to perform the work of a well driller or pump installer. At least 1 reference letter shall be completed by a person who is registered pursuant to the act in the same category as the applicant.(d) Any of the following which verifies that the applicant has acquired the minimum experience and which identifies the applicant's occupation as a well driller or pump installer and states the name of applicant's employer:(i) Federal W-2 income tax withholding forms or equivalent income tax filing forms.(iii) Employers' payroll records.(iv) Workers' compensation insurance records.(v) Health insurance records.(vi) Any combination of the items listed in paragraphs (i) to (v) of this subdivision.(2) Documentation of completion of an apprenticeship training program established by the Michigan well drillers association or a similar industry organization may be accepted in place of the proof of work experience that is required in subrule (1) of this rule, if the training program has been approved by the department and the advisory board created by the act.Mich.
