Section 35.04.01.03 - Application Procedures

An applicant shall:

A. Furnish proof of service or proof of spouse's service;

B. Complete an application form that provides information about the applicant's personal history and financial status;

C. Enclose a copy of honorable discharge papers;

D. Submit an income affidavit;

E. Have the results of a physical exam documented by the applicant's physician on the application form; and

F. Provide the medical director with medical records if the applicant has been hospitalized.
