Section 15.01.06.09 - Requirements for Statement of Disagreement

The statement submitted under Regulation .08 of this chapter shall:

A. Be on pages no larger than 8.5 x 11 inches in size;

B. Use only one side of each page; and

C. Consist of no more than five pages.
