Section 10.64.01.18 - Fees

A. The fees listed in this regulation are not refundable.

B. An initial license shall only be valid until the next renewal period.

C. The fees are as follows:

(1) Initial application fee - $900;

(2) Biennial renewal - $800;

(3) Reactivation fee - $800;

(4) Reinstatement Fee - $900; and

(5) Inactive Status Fee - $100.
