The fees established by the Board are as follows:

A. Address change, failure to notify the Board . . . $50;

B. Application for initial licensure (includes first temporary license and is nonrefundable) . . . $200;

C. Application for second temporary license . . . $50;

D. Continuing Competency Provider Contact Hour review . . . $30;

E. Insufficient Funds . . . $35;

F. Reactivation of license after elective nonrenewal . . . $245;

G. Reinstatement . . . $450;

H. Elective nonrenewal . . . $25;

I. Renewal, late fee (applicable to renewal made after the renewal deadline) . . . $100;

J. Renewal, occupational therapist license . . . $270;

K. Renewal, occupational therapy assistant license . . . $210; and

L. Verification of licensure . . . $20.
