Section 10.44.35.04 - Issuance and Renewal of PermitA. A permit expires on October 1 of the 5th year following the effective date of the permit.B. To obtain a permit, a dentist shall: (1) Submit to the Board an application on a form provided by the Board; and(2) Pay the fee set by the Board in COMAR 10.44.20.C. The Board shall mail a renewal application to each dentist holding a limited dental drug dispensing permit at least 90 days before the expiration of the permit.D. Before the permit expires, an applicant may apply to renew the permit for an additional 5-year term if the applicant: (1) Submits to the Board a renewal application on a form provided by the Board; and(2) Pays the renewal fee set by the Board in COMAR 10.44.20.Md. Code Regs. 10.44.35.04
