Section 10.44.26.13 - Renewal of Licenses

Before a volunteer license to practice dentistry or dental hygiene expires, the licensee may renew it for an additional term if the licensee:

A. Submits a completed renewal application on a form provided by the Board;
B. Completes the continuing education requirements for an active general license to practice dentistry or dental hygiene in the State;
C. Submits proof satisfactory to the Board that the dentist or dental hygienist has donated at least 100 hours of dental or dental hygiene services without compensation;
D. Submits evidence that the dentist or dental hygienist is covered by malpractice insurance as set forth in Regulation .09 of this chapter;
E. Submits documentation of change of name if the name on any of the documents submitted to the Board differs from the applicant's name; and
F. Submits any other documents or information pertinent to licensure required by the Board.
