Section 10.44.24.08 - Renewal of Licenses

Before a retired volunteer license to practice dentistry or dental hygiene expires, the licensee may renew it for an additional term if the licensee:

A. Submits a completed renewal application on a form provided by the Board;
B. Completes the continuing education requirements for an active general license to practice dentistry or dental hygiene in the State;
C. Submits proof satisfactory to the Board that the dentist has donated at least 100 hours of dental services, or that the dental hygienist has donated at least 100 hours of dental hygiene services, without compensation;
D. Provides evidence that the dentist or dental hygienist is covered by malpractice insurance as set forth in Regulation .04 of this chapter;
E. Submits a copy of the licensee's National Practitioner Data Bank file;
F. Submits documentation of change of name if the name on any of the documents submitted to the Board differs from the licensee's name; and
G. Submits any other documents or information pertinent to licensure required by the Board.
