The following fees are established by the Board:

A. Initial application fee:

(1) Dentist . . . $450;

(2) Dental hygienist . . . $275;

B. Limited license fee:

(1) Dentist . . . $225;

(2) Credentials processing (foreign dental school graduate surcharge) . . . $75;

C. Teacher's license fee:

(1) Dentist . . . $225;

(2) Dental hygienist . . . $225;

(3) Credentials processing (foreign school graduate surcharge) . . . $75;

D. Temporary license application fee - dental hygienist . . . $75;

E. Application fee for dentists licensed in another state . . . $450;

F. Application fee for dental hygienists licensed in another state . . . $275;

G. License renewal fee:

(1) Dentist . . . $560;

(2) Dental hygienist . . . $182;

(3) Teacher's license . . . $225;

(4) Dentist late renewal fee (July 1 through July 31 of the year in which the renewal is due) . . . $300;

(5) Dental hygienist late renewal fee (July 1 through July 31 of the year in which the renewal is due) . . . $150;

H. Dental radiation technologist certification fee:

(1) Certification renewal . . . $68;

(2) For the renewal period ending March 1, 2017, a o ne-time 1- year certificate for dental radiation technologists whose certificates end in an even number - $34;

(3) Certification reinstatement . . . $118;

(4) For the period March 2, 2017 through April 1, 2017, for the 2017 renewal period, a late renewal fee for dental radiation technologists whose certificates end in an even number...$25;

(5) On and after April 2, 2017, for the 2017 renewal period, a certification reinstatement fee for dental radiation technologists whose certificates end in an even number... $59;

(6) For the period March 2, 2018 through April 1, 2018, for the 2018 renewal period, a late renewal fee for dental radiation technologists whose certificates end in an even number...$25;

(7) On and after April 2, 2018, for the 2018 renewal period, a certification reinstatement fee for dental radiation technologists whose certificates end in an even number... $59;

(8) Late renewal fee (March 2 through April 1 of the year in which the renewal is due) . . . $50;

(9) Initial application fee for applicants holding an active license or certification to practice dental radiation technology in another state . . . $20;

S. Certificate fee for a treating dentist to treat a patient at a practice location other than their own, at which anesthesia and sedation is administered to the patient:

Y. Limited permit for dispensing prescription drugs:
