Section 10.44.02.03 - Requirements for Limited License to Practice Pediatric Dentistry for Certain Individuals
A. In addition to meeting the requirements set forth in Health Occupations Article, § 4-303.1, Annotated Code of Maryland, with respect to moral character, and in addition to passing any required examination, to qualify for a limited license to practice pediatric dentistry in a public health dental clinic, federally qualified health center, or Maryland qualified health center under Health Occupations Article, § 4-303.1, Annotated Code of Maryland, a graduate of a foreign dental school shall submit the following credentials to the Board:
(1) A completed application on a form provided by the Board;
(2) A photograph of the applicant 3 inches by 3 inches accompanied by a notarized statement stating that "the photograph is a true photograph of me";
(3) A copy of the degree or diploma issued to the applicant by the foreign dental school conferring it, properly authenticated by an official of that foreign dental school authorized to make the authentication;
(4) A copy of the subjects taken and the credits earned by the applicant at the foreign dental school, duly authenticated by an official authorized to make the authentication;
(5) A translation into English of a degree, diploma, or subjects taken which are required to be submitted to the Board, certified by an individual acceptable to the Board, if issued in a language other than English;
(6) A certified letter with a raised embossed seal from the dental licensing authority of each state in which the applicant holds an active dental license or ever held an active dental license indicating that the license is or was in good standing and whether the applicant:
(a) Is being investigated;
(b) Has charges pending against the applicant's license;
(c) Has been disciplined; or
(d) Has been convicted or disciplined by a court of any state or country for an act that would be grounds for disciplinary action under Health Occupations Article, § 4-315, Annotated Code of Maryland;
(7) A copy of the applicant's National Practitioners Data Bank file;
(8) Proof satisfactory to the Board that the applicant has passed parts one and two of the National Board Examinations conducted by the Council of National Boards of the American Dental Association;
(9) Proof satisfactory to the Board that the applicant has successfully completed at least a 2-year pediatric dentistry residency program at a dental school or a hospital authorized by any state and which is recognized by the Board;
(10) Proof satisfactory to the Board that the applicant is either a pediatric dental fellow, or has successfully completed a pediatric dental fellowship at the University of Maryland Dental School;
(11) Proof satisfactory to the Board that the applicant is contractually obligated to provide pediatric dental services for at least 2 years in a:
(a) Public health dental clinic operated by the State or a county or municipality of the State;
(b) Federally qualified health center; or
(c) Maryland qualified health center;
(12) A letter from the health officer of the public health program for which the applicant has contractually agreed to provide at least 2 years of pediatric dental services:
(a) Requesting that the Board waive the education requirements of Health Occupations Article, § 4-302, Annotated Code of Maryland, for the applicant;
(b) Stating that circumstances exist to justify granting the waiver; and
(c) Requesting that a limited license be issued to the applicant;
(13) A letter from the Dean of the University of Maryland Dental School:
(a) Requesting that the Board waive the education requirements of Health Occupations Article, § 4-302, Annotated Code of Maryland for the applicant; and
(b) Stating that circumstances exist to justify granting the waiver;
(14) Two letters of recommendation written by persons acceptable to the Board, which certify to the Board the applicant's:
(a) Good moral character;
(b) Age;
(c) Qualifications;
(d) Background; and
(e) Experience, if any;
(15) A letter from the dean of the dental school at which the license is limited indicating that the applicant possesses sufficient comprehension and communication skills in written and spoken English to enable the applicant to adequately treat dental patients;
(16) Documentation of change of name if the name on any of the documents submitted to the Board differs from the applicant's name; and
(17) Any other documents or information pertinent to licensure required by the Board.
B. A limited license to practice dentistry in this State is valid for 1 year from the date of issuance and is not renewable.
C. An individual who has been in possession of a limited license may reapply for a limited license by submitting a new application to the Board.
