Section 10.32.20.08 - Identification; Notice

A. A licensed perfusionist shall wear an identification tag or a badge which identifies that individual as a licensed perfusionist.

B. Posting of License.

(1) A licensed perfusionist shall :

(a) Keep a copy of the licensee's license in their employment file; and

(b) Make the license available for inspection upon request.

(2) If the perfusionist has multiple places of employment, copies of the license may be kept in accordance with §B(1) of this regulation.

C. Change of Name or Address.

(1) A licensed perfusionist shall notify the Board in writing of any changes in name or mailing address within 60 days.

(2) The Board may impose a fine of $100 for failure to notify the Board of a change in name or mailing address.
