Section 10.07.12.04 - License Procedure

Applicants for licensure shall conform to the following:

A. Any person desiring to open and operate a health care facility within a correctional institution or to continue the operation of an existing health care facility, shall file a written or electronic application with the Department giving the information required by the Office of Health Care Quality;

B. The applicant shall submit a synopsis of the health care program to be offered with the application; each subsequent application shall include a list of changes in the original program.
