Section 144-120-2.H - Specifications of License

2.H.1. Each license issued by the Department shall identify:

a. The name of the Hospice Provider,

b. The name of the Administrator,

c. The geographical location;

d. The type of hospice care services provided;

e. The period of licensure and date of licensure expiration, and

f. The date issued and the type of license.
