The applicant shall submit a letter from the appropriate municipal official(s) that demonstrates compliance with all local ordinances relative to zoning and building codes and a certificate of occupancy, if appropriate, which includes electrical and fife safety code compliance. 2.D.2. The applicant will submit proof of a Clinical Laboratory number (if appropriate) under the Clinical Laboratory Improvement Amendments of 1988. 2.D.3. The applicant will submit proof of a Department of Environmental Protection number, if appropriate, regarding disposition of biomedical waste. 2.D.4.
