Section 144-118-7.A - Admissions, Transfers, Discharges
7.A.1. Admission decisions must be based on a current functional assessment of the client that is conducted and updated by the facility or by outside sources on admission.
7.A.2. On admission the facility shall place in the client's record:
a. Medical history;
b. Current medical status and physical disabilities;
c. Transfer form and Medicaid certification;
d. Physician's plan of care;
e.Preadmission IPP;
f. Reason for referral if emergency;
g. Personal property list, signed and dated;
h. Legal competency status and guardianship, if applicable;
i. Physical examination report from physical completed seven (7) days prior to or forty-eight (48) hours following admission.
7.A.3. If admission is not the best plan, but the person must be admitted, the IDT must:
a. Clearly acknowledge that the admission is not the best plan; and
b. Initiate plans to actively explore alternatives.
7.A.4. A preliminary evaluation must contain:
a. Current background information;
b. Currently valid assessments of:
1. Functional development;
2. Behavior;
3. Social;
4. Health and nutritional status;

To determine if:

(a) The facility can provide for the client's needs; and
(b) The client is likely to benefit from placement in the facility.
7.A.5. Availability of Rules and Procedures

The facility shall make available for distribution to clients and/or their guardians) a summary of the laws, regulations, and procedures concerning admission to, readmission to, and discharge from the facility.
