The trainee shall submit monthly progress reports to the board, signed by the supervising licensed practitioner and the trainee, on a form provided by the board. The trainee and supervising licensed practitioner shall promptly address any deficiencies or concerns with the trainee's progress communicated by the board. Failure to timely submit one or more reports or failure to promptly address deficiencies or concerns may result in the immediate termination of the traineeship.
