Section XIII-301 - Required Documentation for New EmployeesA. Please submit a copy of the "Oath of Office" in order to place a new constable or justice of the peace on the payroll. The secretary of state's office should supply the parish with the completed "Oath of Office" form. This form does not include all the necessary information for placing an employee on the payroll. Please be sure to supply the Department of Public Safety with the employee's name, address, Social Security number, and the date the individual took office.La. Admin. Code tit. 55, § XIII-301
