Section V-3103 - Goals and Services
A.
1. The goal of the LHBPCP is to reduce morbidity and mortality associated with hypertension and its complications (heart disease and stroke) through a comprehensive preventive system. Services are targeted to the high risk population 14 years of age and older. The program provides services in six areas:
a. client education;
b. screening/rescreening;
c. counseling;
d. follow-up;
e. monitoring; and
f. direct referral to physician and hospital for diagnosis and treatment.
2. Additionally, the program provides support services to the medical community in identifying hypertensives and, once diagnosed, assists it in controlling blood pressure by facilitating patient compliance. The program may assist the patient in maintaining the medical regimen provided by his/her physician.
