Section I-5503 - Licensure ProcedureA. Application for license is made by applying in writing to the Licensing and Certification Division, Department of Health and Human Resources, P.O. Box 3767, Baton Rouge, Louisiana 70821. This means that the applicant is requesting that the facility be surveyed for licensure. As soon as possible, a surveyor will visit the facility to determine compliance. At the same time, inspections by the Fire and Health Departments will be requested by the Licensing and Certification Division. Copies of these inspection reports which indicate approval by the Fire and Health Departments must be received by the Licensing and Certification Division.B. Any deficiencies found by the surveyor will be discussed with the administrator and/or the governing body of the facility. Within 10 days after official notification of deficiencies noted in the survey, the administrator and/or governing body must reply in writing as to how the deficiencies will be corrected and the dates on which these corrections will be effected.
