A. Any remittance submitted to DHH in payment of a required fee must be in the form of a company or certified check or money order made payable to the Department of Health and Hospitals.B. Fee amounts shall be determined by DHH.C. Fees paid to DHH are not refundable.D. A fee is required to be submitted with the following: 1. an initial application;2. a renewal application;3. a change of controlling ownership; and4. a change of name or physical address.La. Admin.
