Section XXXVII-1203 - Fixed PlaceA. The license is effective for a fixed place, and for a specific name. Whenever the location or name of the license crematory is changed, a new license shall be obtained and a fee in an amount as established by the board must be paid. All changes of name and./or location must be reported to the board's secretary and the application process must be completed prior to making any changes.La. Admin. Code tit. 46, § XXXVII-1203
