Section XLVII-1713 - Verification of LicenseA. An employer requesting verification of a practical nurse's license shall submit the request in writing to the board accompanied by the required fee.B. A licensee requesting verification of lost/stolen/never received license shall obtain a "verification of renewal" form from the board office, submit the completed form and a certified copy of his/her birth certificate accompanied by the appropriate fee. Upon receipt of the above a verification of license shall be issued.La. Admin. Code tit. 46, § XLVII-1713
