Section XLV-3915 - Application ProcedureA. Application must be made and submitted in a format approved by the board and shall include: 1. proof, documented in a form satisfactory to the board, that the applicant possesses the qualifications set forth in this Chapter, along with a recent photograph;2. certification of the truthfulness and authenticity of all information, representations and documents contained in or submitted with the completed application;3. criminal history record information;4. payment of the fee provided in Chapter 1 of these rules; and5. such other information and documentation as the board may require.B. Upon submission of a completed application a personal interview with a member of the board or a designee may be required as a condition of licensure when: 1. discrepancies exist in an initial application;2. an applicant has been the subject of prior adverse action in any jurisdiction; or3. the board has questions respecting an application response.C. The recommendation of the board member or designee as to the applicant's fitness for licensure shall be made a part of the applicant's file.D. The board may reject or refuse to consider an application which is not complete in every detail. The board may in its discretion require a more detailed or complete response to any request for information set forth in the application as a condition to application consideration.La. Admin.
