Section LXVII-1501 - FormsA. A request to terminate sponsorship of a licensee or to transfer a licensee to a new broker shall be submitted on forms prescribed by the commission and shall be accompanied by the fees prescribed in R.S. 37:1443.La. Admin. Code tit. 46, § LXVII-1501
