Section LXII-1111 - Replacement License/Certificate Credential or License Registration CardsA. A new or duplicate license/certificate credential, or a new registration card to replace one lost, destroyed, or mutilated, may be issued, subject to the rules of the board, upon payment of the established fee(s).La.
