Section LVII-515 - Registration Card

A. The registration card shall be no larger than 2 1/4 inches by 4 inches in size. The registration card shall contain the following information:

1. name of investigator;

2. name of agency under whose authority license is issued;

3. date of expiration;

4. current 2 inches by 2 inches color photograph;

5. drivers license number;

6. company name;

7. company address (city and state);

8. license number;

9. signature of executive director;

10. signature of license holder;

11. state insignia; and

12. board seal.
