Section III-3501 - Patron Dispute FormA. Whenever a licensee or casino operator and a patron are unable to resolve a dispute regarding the payment of winnings, the licensee or casino operator shall provide the patron a patron dispute form.La. Admin. Code tit. 42, § III-3501
