Section V-1111 - Determination and Record of Emergency ProcurementA. Determination. The procurement officer or the head of a purchasing agency shall make a written determination stating the basis for any emergency procurement or award of a contract, and for the selection of a particular contractor. Such determination shall be sent promptly to the chief procurement officer for approval or rejection.B. Record 1. A record of emergency procurement shall be maintained in a form/format determined by the director of the Office of State Procurement that lists, for those entities under its jurisdiction, at a minimum, the following: a. each contractor's name;b. the amount and type of each contract;c. a listing of the supplies, services, major repairs, or professional, personal, consulting or social services procured under each contract; andd. the identification number of each contract file.2. The record for the previous fiscal year shall be provided by the Office of State Procurement at the beginning of the legislative session.La. Admin. Code tit. 34, § V-1111
