Section XV-742 - Safety InstructionsA. The licensee shall provide oral and written radiation safety instruction to all personnel caring for a patient or human research subject receiving brachytherapy and cannot be released under LAC 33:XV.725. Refresher training shall be provided at intervals not to exceed one year.B. To satisfy the requirements of Subsection A of this Section, the instruction shall describe: 1. size and appearance of the brachytherapy sources;2. safe handling and shielding instructions in case of a dislodged source;3. procedures for patient or human research subject control;4. procedures for visitor control, including: a. routine visitation of hospitalized individuals in accordance with LAC 33:XV.421.A.1; andb.visitation authorized in accordance with LAC 33:XV.421.F;5. procedures for notification of the radiation safety officer, or his or her designee, and an authorized user if the patient or human research subject dies or has a medical emergency; and6. LAC 33:XV.Chapter 10 training requirements.C. A licensee shall maintain a record of individuals receiving instruction required by Subsection A of this Section, a description of the instruction, the date of instruction, and the name of the individual who gave the instruction for three years.La.
