Section III-505 - AffidavitsA. Applications and renewal applications must be accompanied by affidavits by each owner, and director unless previously approved, and Solicitor Permit Applications (PSC-4 Form) by each solicitor containing the information prescribed by R.S. 17:3140.13 (for solicitor renewal, see §703). In the case of office and clerical personnel, in lieu of the affidavits of such personnel, the owner may submit an affidavit setting forth the information prescribed R.S. 17:3140.13 concerning such personnel.
