Section 112-104-39 - Corrections to forms
(a) Monetary corrections to a figure originally recorded on a form may be made only in ink by performing the following:
(1) Crossing out the error;
(2) entering the correct figure; and
(3) obtaining the initials of the employee making the change and the initials of the employee's supervisor.
(b) Each nonmonetary correction to a form shall be initialed by the employee making the correction.
(c) Each form that is not prenumbered shall be maintained and controlled by the applicable department manager.
