Rule 701-256.15 - Application for permit

Retailer permits shall be issued only upon application, accompanied by the applicable fee, and made upon forms furnished by the department. The forms shall specify:

(1) The manner under which the retailer transacts or intends to transact business as a retailer.

(2) The principal office, residence, and place of business for which the permit is to apply.

(3) If the applicant is not an individual, the principal officers or members of the applicant, not to exceed three, and their addresses.

(4) Such other information as the director shall require.

This rule is intended to implement 2005 Iowa Acts, House File 339.
