Rule 657-2.14 - Fees for additional license certificates and verification

(1) Only original license certificates issued by the board for licensed pharmacists are valid. Additional original license certificates for licensed pharmacists may be obtained from the board for a prepaid nonrefundable fee of $20 each. The fee shall be considered a repayment receipt as defined in Iowa Code section 8.2.

(2) The board may require the submission of a nonrefundable fee of $15 for written license verification.
