Rule 653-3.5 - Filing of petition

A petition for a waiver must be submitted in writing to the board, as follows:

(1)License application. If the petition relates to a license application, the petition shall be made in accordance with the filing requirements for the license in question.
(2)Contested cases. If the petition relates to a pending contested case, the petition shall be filed in the contested case proceeding, using the caption of the contested case.
(3)Other. If the petition does not relate to a license application or a pending contested case, the petition may be submitted to the board's executive director.
(4)File petition. A petition is deemed filed when it is received in the board office. A petition should be sent to the Iowa Board of Medicine, 400 S.W. 8th Street, Suite C, Des Moines, Iowa 50309-4686. The petition must conform to the form specified in rule 3.17(17A,147,148).
