Rule 645-206.2 - Initial licensure

The following criteria shall apply to licensure. The applicant shall:

(1) Submit a complete online application and pay the nonrefundable fee specified in rule 645-5.11(147,148B).

(2) Submit an official copy of academic transcripts directly from the school to the board. No application will be considered by the board until official copies of academic transcripts have been received.

(3) Direct the examination service to submit examination scores directly to the board.

(4) Provide verification of license(s) from every jurisdiction in which the applicant has been licensed, sent directly from the jurisdiction(s) to the board office. Web-based verification may be substituted for verification direct from the jurisdiction's board office if it provides:

a. Licensee's name;

b. Date of initial licensure;

c. Current licensure status; and

d. Any disciplinary action taken against the license.
