Rule 645-21.17 - Reactivation of a barbershop license

To apply for reactivation of an inactive license, a licensee shall:

(1) Submit a reactivation application on a form provided by the board.

(2) Pay the reactivation fee that is due as specified in 645-subrule 5.2(11).

(3) Meet the requirements for sanitary conditions established in 645-Chapter 22.
