In lieu of the words "(designate office)", insert "Iowa Plumbing and Mechanical Systems Board." A request for access to a record should be addressed to the Iowa Plumbing and Mechanical Systems Board, Lucas State Office Building, 321 E. 12th Street, Des Moines, Iowa 50319-0075.
In lieu of the words "(AGENCY NAME)", the heading of the Petition should read:
"BEFORE THE IOWA PLUMBING AND MECHANICAL SYSTEMS BOARD"
