Rule 641-27.4 - [Effective until 6/19/2024] Official communications

(1) All official communications, including submissions and requests, may be addressed to the Plumbing and Mechanical Systems Board, Lucas State Office Building, 321 E. 12th Street, Des Moines, Iowa 50319-0075.

(2) Notice of change of address. Each licensee and licensed entity shall notify the board of a change of the current mailing address within 30 days after the occurrence.

(3) Notice of change of name. Each licensee shall notify the board in writing of a change of name within 30 days after the occurrence.
