Rule 481-63.6 - Required notifications to the department

The department shall be notified:

(1) Thirty days before any proposed change in the residential care facility's functional operation or addition or deletion of required services; (III)

(2) Thirty days before the beginning of the renovation, addition, functional alteration, change of space utilization, or conversion in the residential care facility or on the premises; (III)

(3) Thirty days before closure of the residential care facility; (III)

(4) Within two weeks of any change in administrator; (III)

(5) Ninety days before a change in the category of license; (III)

(6) Thirty days before a change of ownership. The licensee shall:

a. Inform the department of the pending change of ownership; (III)

b. Inform the department of the name and address of the prospective purchaser, transferee, assignee, or lessee; (III)

c. Submit a written authorization to the department permitting the department to release all information of whatever kind from the department's files concerning the licensee's residential care facility to the named prospective purchaser, transferee, assignee, or lessee. (Ill)
