Rule 441-118.8 - Adverse actions

(1) An eligible applicant must be notified of the right to appeal the rating decision in accordance with 441-Chapter 7.

(2) A participant's quality rating shall be revoked if the facility no longer meets the definition of "eligible applicants."

(3) Form 470-4230, Quality Rating Certificate, shall be returned to the department of human services if:

a. The certificate is revoked;

b. The certificate is not renewed; or

c. The provider voluntarily withdraws from the program.

(4) Ratings are effective for 24 months from the date of issuance.
