Rule 441-4.1 - [Effective 7/3/2024] Petition for rulemaking

In lieu of the words "designate office" insert "Compliance Division, Iowa Department of Health and Human Services, Lucas State Office Building, 321 E. 12th Street, Des Moines, Iowa 50319" and in lieu of the words "AGENCY NAME," the heading on the petition should read "THE DEPARTMENT OF HEALTH AND HUMAN SERVICES".
