Sec. 1.
(a) A renewal application shall be submitted to the licensing agency on or before December 31 of each odd-numbered year on a form provided by the licensing agency. The application shall be accompanied by the following: (1) The renewal fee required by 880 IAC 1-1-5.(2) Evidence of the continuing education required by section 2 of this rule.(b) A person who holds a license in speech-language pathology or audiology, or both, must renew each license biennially as required by IC 25-35.6-3-6.(c) A license that is not renewed on or before December 31 of each odd-numbered year becomes invalid.
