Section 848 IAC 3-2-3 - Photograph submitted with application

Sec. 3.

A recent passport type photograph of the applicant taken within six (6) weeks prior to filing shall be submitted with each application. The photograph shall be in color on semigloss paper, not less than three (3) inches wide and four (4) inches high.
