Sec. 4.
In any case where a person's license or registration has been suspended, said person shall:
(1) Within thirty (30) days from the date of the order of suspension, file with the medical licensing board an affidavit showing that: (A) All active patients then under the licensee's or registrant's care have been notified in the manner and method specified by the board of the licensee's or registrant's suspension and consequent inability to act for or on their behalf in a professional capacity.
